How to make a personal accident claim
 
To make an injury claim, take the following steps within 30 days of the injury occurring:

  1. Obtain a claim form from OAMPS Sport & Leisure by calling (02) 8838 5700 or download it by clicking the link in the claims form downloads box on the right.

  2. Arrange for your doctor to complete a 'medical statement'

  3. Arrange for Victorian Dragon Boat Association's secretary to complete 'the association’s declaration'

  4. If claiming loss of income have the employer complete 'the member’s employment details'

  5. Return the completed claim form to OAMPS Sport & Leisure within 30 days of the injury occurring

 How to make public, products and professional liability claims

It is essential that you notify OAMPS Insurance Brokers immediately on (02) 8838 5700 of any potential claim. OAMPS will then provide you with advice on how to proceed.
