Hockey Queensland has chosen the following cover with OAMPS Insurance Brokers:

Medicare pays all eligible costs. Hockey Queensland's policy then covers such expenses as private hospital accommodation, ambulance, physiotherapy and dental fees up to a combined $3,000 in total for each medical claim.
The level of cover under the program is not 'comprehensive' and, as such, we encourage all participants to review their own individual needs. Where appropriate, we advise participants to take out private health, life and income protection insurance. It is an individual’s responsibility to ensure that he/she has adequate insurance cover for his/her needs.

Both Local Standard Time at the insured’s Australian head office or other commencement date as required to fit in with due date of current policy.
Medical
80% of non-Medicare expenses up to $3,000. Government legislative and legal conditions prevent your insurer from paying Medicare gaps. Under the policy you can claim physiotherapy, private hospital accommodation, ambulance and dental fees. An excess of $50 applies. Physiotherapy expenses are limited to $350 any one claim.
Legal liability
Limit of indemnity $10,000,000 - provides protection for the organisation, first aid officials, coaches, players and others officially involved with the club. Excess $1,000.
Professional liability
Limit of indemnity $2,000,000 - cover applies to coaches and umpires. Excess $1,000.
Loss of income
If you're a wage earner, temporary total disablement of $500 per week is payable up to a maximum of 52 weeks or 80% of your weekly wage, whichever is the lesser. There is a waiting period excess of 14 days.
Student assistance / home help benefits
Reimburses up to $350 per week for a maximum of 52 weeks for costs incurred for tutoring or continuation of education and home duties. Waiting period excess is 14 days.
Parents' allowance
Reimburses $15 for each day the insured child is hospitalised-offsetting costs incurred for babysitters, taxi fares and so on. A one day excess applies.
